A Settlement has been reached with Prime Healthcare Services, Inc. (“Prime Healthcare” or “Defendant”) in a class action lawsuit. This lawsuit concerned the Defendants’ use of third-party tracking technologies, called “tracking pixels,” on its web pages. Plaintiffs claim that using tracking pixels violated certain state and federal privacy laws.
The lawsuit is captioned Doe, et al. v. Prime Healthcare Services, Inc., et al., Case No. 23STCV26800, pending in the Superior Court of California for Los Angeles County (the “Action”).
The Defendants deny they did anything wrong, and the Court has not decided who is right.
The Parties have agreed to settle the lawsuit (the “Settlement”) to avoid the costs and risks, disruptions, and uncertainties of continuing the litigation.
You are included in the Settlement Class if you were a patient of Prime Healthcare and the Relevant Hospitals and you accessed one of Prime Healthcare’s Websites between November 1, 2019, and October 18, 2023(see Question 5 of the FAQ for a list of Relevant Hospitals and Websites).
Your rights are affected whether you act or don’t act.

Important Dates

Claims Deadline
Monday, December 21, 2026
You must submit your Claim Form online no later than Monday, December 21, 2026, or mail your completed paper Claim Form so that it is postmarked no later than Monday, December 21, 2026.
Exclusion Deadline
Monday, December 21, 2026
You must submit your written Request for Exclusion so that it is emailed or postmarked no later than Monday, December 21, 2026.
Objection Deadline
Wednesday, January 20, 2027
You must submit your objection so that it is emailed or postmarked no later than Wednesday, January 20, 2027. You may also object in person at the Final Approval Hearing.
Final Approval Hearing
Tuesday, March 9, 2027
The Final Approval Hearing is scheduled for Tuesday, March 9, 2027 at 10:00 a.m. Pacific Time at the Spring Street Courthouse of the Superior Court of California, County of Los Angeles , at 312 North Spring Street, Dept. 7, Los Angeles, CA 90012.

SUMMARY OF YOUR LEGAL RIGHTS AND OPTIONS IN THIS SETTLEMENT		DEADLINE
SUBMIT A CLAIM	Settlement Class Members who accessed one of Prime Healthcare’s Patient Portals between November 1, 2019 and the Class Period End Date for the relevant Prime Web Property in the table below (“Patient Portal Subclass Members”), or were a patient at one of Prime Healthcare’s California hospitals between November 1, 2021, and the Class Period End Date for the relevant California hospital, which you may review at the table below, and provided Prime Healthcare with your email address (“Email Subclass Members”) automatically receive a code for one (1) year of CyEx Medical Shield Pro with their Notice—no Claim Form is required. Patient Portal Subclass Members may also receive a $20 cash payment by submitting a valid and timely Claim Form. The fastest way to submit your Claim Form is online by using the Submit Claim button on this Settlement Website. If you prefer, you can download the Claim Form from the Important Documents page of this Settlement Website and mail it to the Settlement Administrator. You may also call or email the Settlement Administrator to receive a paper copy of the Claim Form.	December 21, 2026
OPT OUT OF THE SETTLEMENT	You can choose to opt out of the Settlement and receive no Settlement benefits. This option allows you to sue, continue to sue, or be part of another lawsuit against the Defendants related to the legal claims resolved by this Settlement. OBJECT TO THE SETTLEMENT AND/OR ATTEND A HEARING	If you do not opt out of the Settlement, you may object by sending a written objection to the Settlement Administrator explaining why you think the Court should not approve the Settlement, or by objecting in person at the Final Approval Hearing. You may also ask the Court for permission to speak about your objection at the Final Approval Hearing. If you object, you may also file a claim for a cash payment (if you are a Patient Portal Subclass Member) and activate your code for CyEx Medical Shield Pro (if you are a Patient Portal Subclass or Email Subclass Member). DO NOTHING	Unless you opt out of the Settlement, you are automatically part of the Settlement. If you do nothing, you will not receive a cash payment (although you may still use the CyEx Medical Shield Pro code included with your Notice), and you will give up the right to sue, continue to sue, or be part of another lawsuit against the Defendants related to the legal claims resolved by this Settlement.
